Become a Patient at FMF
Step #1: New Patient Inquiry
To start the process of becoming a patient with Functional Medicine Foundations, please complete the New Patient Inquiry Form below. After submitting the form, you will receive a New Patient Inquiry Call from an FMF Client Relations Specialist. During the New Patient Inquiry Call, a Client Relations Specialist will discuss our patient process, billing questions, and answer any questions you may have.
Step #2: Patient Account Registration
Functional Medicine Foundations uses an online portal to track all patient records, appointments, and communications. Immediately after your New Patient Inquiry Call, you will receive an email containing the link to finish setting up your account in the Patient Portal. Once you’ve logged in to your patient portal, please complete all intake questionnaires located under “Questionnaires," and upload an image of the patient and insurance card under “Labs & Documents."
Step #3: Scheduling Call
Once you’ve submitted all intake forms in the patient portal, you will receive a Scheduling Call from an FMF Client Relations Specialist
this Scheduling Call, a Client Relations Specialist will schedule your Provider Intake Visit, address any missing information in your patient portal, and answer any questions you have.
